Mariah will assist our patients with chronic conditions like high blood pressure and diabetes with care coordination outside of their clinic visits. By performing weekly home visits, facilitating communication with their providers, and connecting them to supportive resources to overcome barriers and build a healthy community. Every day, Mariah will be promoting better health through individual interactions and by supporting our unique programs for nutrition, exercise, cancer screenings, and more.
“My primary purpose in life is helping others – the same way someone once helped me,” Mariah says. She brings to her work past experience in retail, caregiving, and as a peer support specialist, as well as college-level studies in criminal justice and nurse assisting. She holds an adult peer support specialist certification in substance use disorders/mental health disorders through Mental Health of America.
