Huntington, WV, June 27, 2012-- Since 1976, The State Health Education Council (SHEC) of WV has been bringing together individuals and organizations from around the state of West Virginia to build a network of people committed to promoting positive lifestyle behaviors among all West Virginians. The 33rd Annual SHEC Conference was held June 4 – 6, 2012 at the Bridgeport Conference Center in Bridgeport WV. During the President’s luncheon and Awards banquet, the SHEC Outstanding Organizational Leadership award was presented to Ebenezer Medical Outreach in recognition of excellent service and commitment to health promotion activities in West Virginia. Special Needs Registry System

The Cabell-Huntington Health Department is developing a registry for people in the community who have special needs. This information will assist emergency management agencies in planning to meet the needs of people before, during, and after emergencies. A person with special needs is someone who needs daily medication, or insulin, or oxygen, or need some sort of medical equipment that requires electricity. The complete list of what is considered special needs can be found on the website form or the paper form.

If you or someone you know has special needs, please take the time to complete the Individual System Form. This form can be found online at www.helpmcwv.org or a paper application can be obtained from the health department and then returned to the health department. After you turn in the form, someone from the health department will be calling to verify your information. The health department will also be calling periodically to update your information.

The information collected will not be available to the public. It will only be shared with public safety agencies to improve their ability to plan for the special needs of the people that they serve. A full HIPAA Disclosure Notice is available upon request.

Please understand that completing this form does not mean that you will be served first in the event of an emergency. It is very important that you have your own personal emergency plan. The information on this form helps public safety agencies to prioritize the needs of people in the event of an emergency.

The website also registers people and places that would like to volunteer their services and/or facility to assist people with special needs during emergency events. Please complete the Facility System Form and/or the Volunteer System Form if you are interested.
